Ashland Dental Arts accepts most PPO and indemnity plans. Our list of in-network providers is at the bottom of this page. We always recommend that you verify your coverage and network status with your insurance company prior to your visit. The best way to reach your dental insurance company is by calling the phone number on the back of your insurance card or by using your online member portal. Please keep in mind that out-of-network insurance may not cover as much of your visit compared to in-network insurance.

Per your contract and our office policy, your deductible and coinsurance are due at time of service.
